Our position enables us to highlight issues and concerns honestly without prejudice, fear of reprisal or personal conflict. We present the facts as they are and transfer the ownership to those accountable. The final action resides with the individuals addressing the root causes and implementing permanent corrections.
We have all had our initial Human Factors (HF) training and left our course with a “WOW” factor. But honestly, how long does that reality check truly last? Will it last until the next recurrent human factors course? Perhaps, but let’s be sincere, most of us are relatively quick to unwittingly fall back into the HF trap and to slowly decrease our alertness to surrounding threats, undermining our ability to address them. Without any doubt, as part of a Safety Management System (SMS), the independent Propulsion Bound influence is more than beneficial.
While the responsibilities relative to aviation maintenance and surveillance have been, over the years, perceived as two independent entities, they are essentially the same. Actually, those involved and those responsible for providing a safe and reliable product have not changed. Thus, they are still the ones responsible for implementing the means necessary to insure an equivalent, safe and reliable, end result.
With reference to the Transportation Safety Board Watchlist 2012: “...highlighted gaps in TC’s planning and implementation as well as oversight of SMS operators. In addition, recent investigations by the Transportation Safety Board of Canada (TSB) have highlighted difficulties faced by operators in transitioning from traditional safety management to SMS.”
